How you’ll make an impact in this role

• Creates a plan for care across the continuum, integrating patient/family preferences and values.
• Serves as a resource to assess, evaluate, recommend and treat legal, neglect, substance abuse, crisis, psychosocial and various adjustment needs.
• Advocates for resources and removal of barriers.
• Maintains ongoing dialog with supervisor and care transition team members to ensure effective implementation and reevaluation of health plan.
• Acts as a resource for adequate medical record documentation, complex level of care recommendations, and services as they relate to diagnoses, and treatment options for post-discharge care.

What minimum requirements you’ll need

Licensure / Certification / Registration:

Required Credential(s):

One or more of the following:

• Licensed Clinical Counselor credentialed from the Kansas Behavioral Sciences Regulatory Board obtained prior to hire date or job transfer date.
• Licensed Counselor credentialed from the Kansas Behavioral Sciences Regulatory Board obtained prior to hire date or job transfer date.
• Licensed Clinical Social Worker credentialed from the Kansas Behavioral Sciences Regulatory Board obtained prior to hire date or job transfer date. OR, Licensed Bachelor Social Worker (BSW), OR Licensed Master Social Worker (MSW) credentialed by the Kansas Behavioral Sciences Regulatory Board.

Preferred Credential(s):

• Case Manager credentialed from the American Case Management Association (ACMA).
• Case Manager credentialed from the Commission for Case Manager Certification (CCMC).

Education:

• High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.
