Nurse Practitioner, Heart Success Transplant Coordinator- Atlantic Health Morristown Medical Center

• Lead the coordination and delivery of comprehensive pre- and post-heart transplant care across inpatient and outpatient settings. 

• Manage transplant candidate evaluations, waitlist activities, and post-transplant follow-up to ensure timely, patient-centered care. 

• Collaborate with physicians, advanced practice providers, nurses, and subspecialty teams to optimize clinical outcomes and continuity of care. 

• Oversee transplant program operations, including workflow development, protocol implementation, and patient care processes. 

• Monitor quality metrics, clinical outcomes, and regulatory requirements to support program excellence and continuous improvement. 

• Maintain transplant registries, reporting systems, and program data to ensure accurate tracking and analysis of patient outcomes. 

• Supervise and support transplant nursing and clinical support staff, fostering collaboration, accountability, and professional development. 

• Coordinate patient education, testing, consultations, and transitions of care throughout the transplant journey. 

• Develop and maintain strong relationships with transplant centers, referring providers, and key clinical partners to support program growth. 

• Participate in direct patient care activities, including assessment, care planning, documentation, and clinical decision-making as needed.
