Home Health Nurse I (RN): Hoag at Home

Primary Duties and Responsibilities 

The Home Health Nurse I (RN)role provides direct patient care in accordance with the Nurse Practice Act to assigned patients. The nurse collaborates with other disciplines of the health care team to ensure the delivery of safe, timely and appropriate quality patient care. This includes providing physical assessments, nursing interventions, pain management, medication administrations, specimen collection, meeting nutritional needs, and teaching and discharge needs of the patients assigned. Utilizes the nursing process based on evidence-based best practices to meet patient care needs. Evaluates and adjusts the nursing care process as necessary with consultation as needed, and confers with the appropriate resource personnel when making ethical/legal/medical dilemma decisions. Performs other duties as assigned.

Float: Provides care in both the home health and hospice settings. Assignments will be throughout Orange County, patient care territories and assignments may change daily.  Service may cover a large geographic area.

Behavioral Health Nurse (RN): Utilizes medical/ psychosocial model to individualize care plans based on Physician orders for patients with depression, anxiety, or dementia, with the goal of keeping patients in their homes and out of the hospital. Performs comprehensive behavioral health evaluations of mental status, mood, and behavior.

Qualifications

Education and Experience

• Home Health: One (1) year of clinical nursing experience required. One (1) year of home health care experience preferred. Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.

License Required

• Current licensure as a Registered Nurse in the State of California.
• Current Driver's License

Certifications Required

• Current BLS certification prior to clinical rotation.
